Personal Disclosure Form

Hi all!

Is there anyway of reading what the RCMP Personal Disclosure form entails?
Can one download this form from the internet?

I have seen a form with a similar description on the main RCMP website but requires a viewing program called ICS. Is the ICS Viewer free to download for any user? and if so how must one register the user for the program?

Any advice will be appreciated as I simply want to have a look at the type of questions the PDF comprises of.

ICS is a RCMP computer program to review different forms. Contact your local recruiting office or detachment and they should be able to get you a copy of that form!!!
